This module provides a solid foundation for the management of older adults from the APN perspective. It focuses on assessment, management and evaluation of care and services from a holistic and multidisciplinary approach.
Upon completion of this module, students should be able to:
Understand the basic science and gerontology in aging
Perform comprehensive geriatric assessment, functional assessment and simple cognitive screening for older adults including physical examination.
Apply the principles of management in older adults presenting with common geriatric problems (syndromes) inpatient, outpatient and community setting
Appreciate the concepts of person-centered care in Dementia care.
Analyze the importance of the role of APN in collaboration with the interdisciplinary team members in geriatric care.
Understand the principles of geriatric pharmacology and prescribing for older adults
Comprehend the principles of rehabilitation in older adults
Apply the concepts of case management and discharge planning with awareness of community services in Singapore to right siting and safe transfer of the older adults to community, intermediate and long term care.
Examine the role of the informal carers in managing older adults at home and in the community.
Recognize the ethical and legal issues in care of the vulnerable older adults
